Babies can smile very early in life, and fetuses can even engage those reflexes in utero, says Mark Gettleman, MD, a pediatrician and owner of Dr. Goofy Gettwell Pediatrics in Scottsdale, Arizona. These first smiles aren't intentional or in response to anything.
